Social Worker (Adult social care)

Care and Support provide a holistic and integrated service for adults aged 18 and over. We support individuals with both physical and mental health needs, working closely with a wide range of partner agencies; including drug and alcohol services, mental health practitioners, health professionals, housing services, the police, and children’s social care, to ensure coordinated and effective support.

Social Worker - Approved Mental Health Professional (AMHP)

Our dedicated AMHP team is led by a single point of contact for all daytime Mental Health Act assessments across the county, ensuring consistency and collaboration.

Follow patients through their full journey, from hospital admission to discharge and reintegration into the community. Work four days per week, including one 8.00am – 8.00pm shift.
